The Home First Project Coordinator will work with the Patient Flow and interprofessional practice teams with a focus on reducing avoidable ALC days in hospital. The primary focus of this role will be to oversee and manage the Home First Project including refresh the Home First Philosophy. Collaborating with the Hills of Headwaters Ontario Health Team to provide seamless transitions from hospital to home. This position will work on both process development and relationship building.

Duties:
  • Following the hospitals assessment, develop, implement and prioritize the ALC best practice initiatives and ensures that appropriate standards are met which leads to positive patient outcomes.
  • To optimize hospital capacity and patient flow by applying best practices in admission avoidance and facilitating removal of obstacles to discharge.
  • Participates in the development of ALC written philosophy, objectives, and standardize compatible process with the vision, values and mission of the organization.
  • Promotes ways to solve problems and make improvements in the delivery of patient care and barriers to discharge.
  • When needed assists with the coordination of patient flow from inpatient to home or the Emergency department to home.
  • Participate in daily discharge rounds to understand organizational flow and barriers
  • Refresh the “Home First” philosophy across all HOH OHT member agencies and physicians.
  • To optimize hospital capacity and patient flow by applying best practices in admission avoidance and facilitating removal of obstacles to discharge.
  • Work with multi-organizational teams to identify and prioritize opportunities to further design, develop and/or improve future state processes and standards of work related to discharge planning and ALC avoidance.
  • Assist in the development of linkages with community services
  • Leads the development of admission and discharge packages.

Qualifications & Competencies:

  • Valid registration and in good standing with a Professional Healthcare Regulating Body;
  • Completed Baccalaureate Degree in a health-related discipline; or actively pursuing and a commitment to continuous learning
  • Knowledge and experience with Long Term Care strategies, ALC designation and discharge process
  • 3-5 years hospital experience required
  • 1 year experience in the last 5 years liaising with community partners
  • Ability to work with a diverse stakeholder group with demonstrated facilitating and coaching clinical areas on change initiatives
  • Self-directed, courageous, and highly motivated with excellent interpersonal and communication skill
  • Experience managing competing demands in a fast-paced, multi-disciplinary health care environment
  • Evidence of effective communication and conflict resolution skills with the ability to develop positive working relationships with all stakeholders promoting collective problem solving approaches;
  • Excellent interpersonal skills
  • Project Management experience an asset
