We are looking for a Team Lead to work with our Ambulatory Care Department to help us continue in our mission to serve our community with Kindness, Passion, Courage, and Teamwork

Position Responsibilities:
The successful applicant will perform the duties expected of a Team Lead within the Ambulatory Care Department

The Team Lead is a leadership role with accountabilities and responsibilities for coordinating and organizing the daily operation of the nursing care unit. The Team Lead ensures evidence-based, safe, and consistent patient care services are delivered on a daily basis, according to the Standards of Nursing Practice of the College of Nurses of Ontario, and approved standards of care of the Corporation.

Operational
  • Manages patient flow, in collaboration with Patient Care manager, to determine staffing requirements, staff allocation and reassignments based on the needs of the department and patient care needs. Communicates these requirements in order to ensure needs are met.
  • Works with leadership team to ensure all hospital and unit resources are aligned to support the successful achievement of patient care access and delivery
  • Leads and participates in unit meetings, unit committees and special projects
  • Communicates information from leadership to staff
  • Leads Huddles when required
  • Ensures occupational health and safety standards and legislation are followed and met
  • Assists with the orientation/education of new employees
  • Acts as a role model for staff
  • Supports staff with workload concerns and determine strategies to meet patient care needs using current resources

Quality and Safety
  • Monitors unit specific data for trends, identifying opportunities for improvement. Leads and evaluates quality improvement initiatives
  • Recommends and participates in the creation and revision of policies and procedures
  • Timely follow-up on Safety & Quality Information System (SQIS) low and moderate risk reports
  • Participates in the development of objectives for the unit.
  • Promotes an environment that encourages and supports change using change management theory
  • Ensures that the Standards of Nursing Practice are implemented and assists in monitoring of standards as necessary
  • Promotes Bed Side Shift Reporting
  • All employees of Headwaters Health Care Centre (HHCC) are responsible to contribute to a transparent culture of patient and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by HHCC.

Staff Role Modelling
  • Supports and mentors staff in the provision of exceptional patient care
  • Fosters a positive team environment, addressing interpersonal and interdisciplinary conflicts that arise using conflict resolution skills
  • Demonstrates a patient centered approach in all interactions
  • Assists in the planning and coordination of educational opportunities for professional development of staff
  • Acts as a resource to staff with identified learning needs (e.g. professional, technical)
  • Ensures adherence to HHCC policies and procedures and provides direction to staff
  • Attends and actively participates in required in-service education programs.

Patient Care

  • Acts as a liaison to leadership team and facilitate communication among the intra-professional care team, other departments, physicians and all other disciplines and partnerships with other organizations
  • Ensures that referrals are made to appropriate members of the health care team members
  • Leads discharge promotion rounds daily
  • Evaluates and maximizes use of individual care plans, process plans, discharge care plans and transfer tools along with assigned nurse to meet the needs of individual patients and minimize their length of hospitalization
  • Promotes a patient centered philosophy of care.
  • Ensures that the patient's present/potential problems are identified from the patient assessment: patient's health status is determined, health status is compared to the norms, problems are given priority according to impact upon the patient's health status and desire outcomes are formulated specific to the patient problems and established norms.
  • Acts rapidly and effectively, follows hospital policies and procedures in any emergency situation, and utilizes principles of management in any emergency situation.
  • May be required to assume a full patient assignment.
  • Can care for all patient populations attending Ambulatory Department Clinics
  • Promotes an environment that encourages and supports change using change management theory

  • Performs other related duties as assigned.

Qualifications:

  • Current registration as a Registered Nurse with the College of Nurses of Ontario
  • Bachelors’ Degree in Nursing (required)
  • Completion of a De Souza -Provincial Standardized Chemotherapy and Biotherapy Course (PSCB)or equivalent from an accredited college or university (required)
  • Minimum of 5 years relevant clinical experience in Oncology preferred (required)
  • Certification in BCLS (required), ACLS (preferred),
  • RNAO membership (preferred)
  • Demonstrates effective leadership behaviors including the ability to coach and mentor others, conflict resolution, multi-tasking, critical thinking, and effective problem solving
  • Knowledge and application of Lean methodology (preferred)
  • Effective interpersonal, communication and organizational abilities to promote teamwork with staff, physicians and the interdisciplinary health care team.
  • Ability to relate well with patients and families and respond to concerns.
  • Experience in staff scheduling and knowledge of API (required)
  • Ability to organize and prioritize patient care activities with flexibility and adaptability to changing situations
  • Knowledge of Occupational Health & Safety Act and relevant legislation (an asset)
