We are looking for a Primary Care Network (PCN) Coordinator to work with our Ontario Health Team (OHT) to help us continue in our mission to serve our community with Kindness, Accountability, Respect, and Teamwork. 
 
The PCN Coordinator will be responsible for providing support and structure to ensure the effective development of a local primary care network across Dufferin and Caledon. This role involves working with and supporting an emerging PCN Advisory Committee (currently lead by a group of local family physicians), implementing strategic initiatives, managing resources, and facilitating collaboration with amongst primary care providers. 
 
This role is crucial for ensuring the effective development and coordination of PCN activities, improving knowledge sharing across a broad range of physicians, and fostering collaboration within the network. 
 
Position Responsibilities:
 

Leadership and Management Support

  • Provide administrative and operational support to the PCN
  • Where directed, attend Work Groups and Committees to ensure physicians are aware of OHT activities
  • Support recruitment and Continued Medical Education events.

Coordination and Collaboration

  • Facilitate engagement and the development of effective partnerships.
  • Coordinate the development and maturity of the PCN, aligned with provincial reporting.
  • Where directed, attend local and regional planning tables, ensuring members are aware of initiatives and activities of the OHT.

Implementation

  • Track the implementation of PCN’s strategic initiatives and projects.
  • Monitor and enable quality assurance project outcomes.
  • As necessary, ensure that PCN priorities are accurately reflected and considered in broader OHT planning.

Support and Navigation

  • Provide coordination and navigation for primary care providers that are part of the network.
  • Act as a contact for the PCN.
  • Support digital initiatives and maintenance of physician/provider databases

Day-to-Day Activities

  • Attend and support PCN and related meetings.
  • Coordinate, action and report back to PCN leadership on activities and priorities.
  • Where directed, attend OHT Work Group meeting and committees
  • Facilitate information sharing across all models of care within the Caledon and Dufferin communities.

Reporting

  • This is a focused role that will support the PCN Advisory Committee and will therefore report to this committee over time. As physician leadership roles are defined and identified, this reporting may shift.
  • Other reporting activities related to payroll, other human resources related activities will be supported by the OHT leadership, which is part of the Headwaters Health Care Centre.
 
Qualifications:
 
  • Post secondary education in business administration or related field
  • Minimum two years of experience, preferably in the not-for-profit sector.
  • Proficiency in Microsoft Office products including Outlook, Teams, SharePoint, Word, and PowerPoint.
  • Strong organizational skills with ability to coordinate multiple overlapping projects.
  • Excellent communication and interpersonal skills (written, verbal) with a variety of stakeholders.
  • Experience working with providers and clinicians.
  • General knowledge of primary care services and health care systems.
  • Ability to work collaboratively with various stakeholders.
  • Ability to handle multiple and conflicting priorities effectively.
  • Ability to receive directions and feedback from multiple managers.
  • Ability to coordinate meetings and events for large groups.
  • Demonstrated problem solving and critical thinking skills.
  • Ability to work independently.
 

Hiring Salary Range: Compensation is commensurate with experience
 
The duration of this temporary contract is approximately twelve (12) months.
