We are looking for a Registered Nurse to work with our Nurse Led Outreach Team (NLOT)  to help us continue in our mission to serve our community with Kindness, Accountability, Respect, and Teamwork.
 
The Registered Nurse in the Nurse-Led Outreach Team (NLOT) has expertise in geriatrics and seniors’ mental health who provides timely access to evidence based, quality care for older adults living with complex health conditions in the Long-Term Care (LTC) setting. This team provides mobile urgent assessment to residents in their LTC homes for acute and episodic conditions and responds to changes in condition and functional decline to support palliative and end-of-life care in a consult model. NLOT programs reduce unplanned transfers to emergency departments (ED) and hospital admissions, while providing care in the home aligned with resident goals, values and wishes.

Reporting to the Manager, Medicine, CCC and Allied Health, the Registered Nurse (RN), as a member of the Nurse-Led Outreach Team (NLOT), will expand capacity in the LTC homes among staff and clinicians by provide education and training on various relevant topics. This individual will also provide time-sensitive assessments in the LTCs to reduce preventable transfers to the ED, hospital admissions, and reduce hospital LOS. The NLOT RN will identify and facilitate direct access to the right ambulatory services based on assessed needs by working collaboratively with an inter-professional team, family members, support network, community partners, and a variety of informal and formal service providers.
 
An additional focus for the RN will be on both wound care and preventing pressure injuries especially for those long term care, fragile and at high risk patients who are admitted to hospital to ensure that patients are not returned to their homes with a pressure injury. 
 
The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the HHCC mission, vision and values, policies and procedures; and applicable specialty standards. The RN provides patient/client-centered and evidence informed care to promote health and assess the need for the provision of care and the treatment of health conditions. The RN collaborates with the inter-professional team and assumes a leadership role in the delivering a full range of care and treatment to patient/clients including those with complex needs, high acuity and less predictable outcomes.
 
Qualifications:
  • Current registration with the College of Nurses of Ontario required
  • Current BCLS/HCP certification
  • RNAO, CNA or other professional association membership preferred
  • Minimum 3 years of recent related clinical experience working with older adults
  • Training in Comprehensive Geriatric Assessment is preferred
  • Strong clinical assessment skills required.
  • Previous experience in Emergency Department, Stepdown Unit, or Acute Care is an asset.
  • Valid driver's license, adequate insurance (minimum of $5 million personal liability) and use of a reliable vehicle required.
  • Demonstrated ability to utilize principles of knowledge transfer to facilitate practice change & implementation of best practice
  • One of the following:
                  Gentle Persuasive Approach training preferred.
                  CPI – Crisis Prevention Training preferred.
                  MOAB – Preferred
                  Wound Care – preferred
  • Knowledge of best practices of pressure injury prevention
  • Comprehensive knowledge of community resources and health care delivery systems in acute and community settings.
  • Demonstrated knowledge, sensitivity and expertise in the care of seniors and their families.
  • Demonstrated ability to respond to clients and situations with flexibility and adaptability.
  • Demonstrated expertise and competence in areas of client need, including: chronic disease management, mental health and addictions, complex and dysfunctional family systems, older adults who are frail and/or palliative, medication and symptom management for specific diseases.
  • Excellent interpersonal, communication, organizational and decision-making skills required.
  • Demonstrated leadership skills, including problem solving, critical thinking, conflict resolution and negotiation
  • Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers
  • Excellent time management and stress management skills required.
  • Demonstrated knowledge of and experience in the application of adult education principles preferred.
  • Knowledge of the Occupational Health and Safety Act and Regulations, professional standards and guidelines, SJHH corporate and departmental Policies and Procedures, including successful completion of WHMIS training required.
  • Knowledge of documentation standards, privacy legislation, and health record management processes required.
  • Computer literacy and basic proficiency is required.
  • A valid driver's license, adequate insurance (minimum of $5 millions personal liability) and use of a reliable vehicle required.

Hiring Salary Range: $40.24 - $57.68

The duration of this temporary contract is approximately 6 months with the possibility of extending.
